While we make every effort to make this Handbook as clear as possible final interpretation of any section of this Handbook is delegated to ERVA staff and ultimately to the ERVA Executive Board.

To be submitted online by all registered chaperones.

Chaperones submit to Club.  NOT sent to Region Office.

Form that must be filled out by all Club Directors each season.

This form must be presented to ALL Washington state schools that your club uses for practices and tournaments, along with a certificate of insurance, in order to be in compliance with the Lystedt Law concerning head-injury/concussion information.

Submit to WA state schools

Form that is used when there is an injury or property damage that occurs at a practice or sanctioned tournament.
Physical injuries MUST also complete the Medical Claim Form. Region Office will send to Club Director for distribution to claimant.

Coach or tournament director must fill this out for any injury and submit to the Region Office ASAP.

Description of USAV Member Insurance Coverage

Informational ONLY

Form presented to player when being offered a position on a team. This agreement is a binding agreement between players, parents and clubs.

Players complete this and give to club.

Minor Athlete Abuse Prevention (MAAP) Policies each club is REQUIRED to enforce & abide by. This handbook can be included in their own club handbook or linked on the clubs website for reference as well.

Form that contains medical information for each player and MUST be carried by the coach to all training and competition.
